STEP 1 - SUBCONTRACTOR PACKET

Email global@smcarefree.com to request our Subcontractor Packet. Please fill out that packet in full and return it to us via email. You can also snail mail it to our office address below:

STEP 2 - INSURANCE

Please be sure to attach or include a copy of your Certificate of Insurance (COI) to your packet for us to keep on file with your partner account.
STEP 3 - WE WILL BE IN TOUCH!
